What clinical research actually shows

Glucosamine's evidence for knee osteoarthritis presents a genuine, unresolved disagreement between different types of evidence. Multiple meta-analyses report real, statistically significant benefit: one systematic review found significant reductions in pain and improved function, and a large network meta-analysis of 30 trials (5,265 patients) confirmed measurable pain relief, particularly in combination formulations.

However, the landmark GAIT trial — a large, NIH-funded, rigorously designed RCT published in the New England Journal of Medicine — found glucosamine did not significantly outperform placebo overall, with benefit appearing only in a subgroup with moderate-to-severe pain. The American College of Rheumatology's guidelines conditionally recommend against using glucosamine for knee osteoarthritis, even as most physicians in some countries continue to recommend it. This genuine tension between meta-analyses, a landmark trial, and official guidelines is why "Limited-Moderate" is the honest, transparent rating.
