Glucosamine's evidence for knee osteoarthritis presents a genuine, unresolved disagreement between different types of evidence. Multiple meta-analyses report real, statistically significant benefit: one systematic review found significant reductions in pain and improved function, and a large network meta-analysis of 30 trials (5,265 patients) confirmed measurable pain relief, particularly in combination formulations.
However, the landmark GAIT trial — a large, NIH-funded, rigorously designed RCT published in the New England Journal of Medicine — found glucosamine did not significantly outperform placebo overall, with benefit appearing only in a subgroup with moderate-to-severe pain.
