Frankincense has one of the most historically significant trade histories of any remedy in this Atlas — its resin was a cornerstone of ancient trade routes connecting the Horn of Africa and Arabian Peninsula to Egypt, Rome, and beyond, valued as highly as gold in some periods, and famously among the gifts presented to the infant Jesus in the biblical nativity narrative. It also held a distinct medical tradition in Ayurveda, where Boswellia serrata resin was used for arthritis, respiratory complaints, and digestive issues.
Frankincense's modern clinical research has focused specifically on its boswellic acid compounds, particularly for inflammatory joint conditions — an application with genuinely solid trial support, distinct from its better-known ceremonial and aromatic uses.
Frankincense (as Boswellia serrata extract) has real, replicated randomized controlled trial evidence for knee osteoarthritis specifically. A double-blind, placebo-controlled trial (48 patients) using a standardized extract over 120 days found significant improvement in physical function, pain, and stiffness, with radiographic assessments showing improved joint measures. A separate trial found a topical frankincense oil preparation (80 patients) significantly improved knee function and swelling after four weeks compared to placebo.
A broader systematic review of clinical trials using Boswellia serrata for any medical condition identified seven qualifying studies spanning asthma, rheumatoid arthritis, Crohn's disease, osteoarthritis, and collagenous colitis, and found that results across all trials indicated Boswellia extracts were clinically effective for their respective conditions. Given multiple independent RCTs with consistent positive results, particularly for knee osteoarthritis, "Moderate" is the accurate rating.
