Shiitake has been cultivated in Japan for over a thousand years and remains one of the most widely consumed culinary mushrooms globally, valued in East Asian cuisine and traditional medicine alike. Its key studied compound, lentinan, was isolated and became the subject of substantial Japanese pharmaceutical research, similar in spirit to turkey tail's PSK development covered elsewhere in this Atlas.
Shiitake occupies a useful middle position in this Atlas's mushroom coverage: more human clinical trial evidence exists for it than for chaga, though its evidence base remains smaller and more modest than turkey tail's more extensively studied PSK compound.
Shiitake has real, published human clinical trial evidence for immune function specifically. A 4-week randomized dietary intervention found healthy adults consuming 5-10g of dried shiitake daily showed increased activity of natural killer cells and other immune markers, along with reduced C-reactive protein — a genuine human trial finding. For cholesterol, a double-blind randomized clinical trial in adults with borderline high cholesterol found a modest but statistically significant 10% reduction in triglycerides after 66 days, though the trial also noted dermatitis occurred in 10% of participants.
The important caveat, emphasized by Memorial Sloan Kettering's integrative medicine database: most of shiitake's more dramatic anticancer research involves lentinan given by injection, not oral consumption of the mushroom itself, and one clinical trial specifically found oral shiitake extract ineffective for prostate cancer treatment. Given genuine positive human RCTs for immune markers and modest cholesterol effects via ordinary oral consumption, but a clear distinction needed between injectable lentinan research and the culinary mushroom, "Moderate" is the accurate, appropriately scoped rating.
Fresh or dried mushrooms cooked in soups, stir-fries, and broths; dried shiitake is a defining ingredient in East Asian cooking, prized for its concentrated umami flavor.
