Pistachios have been cultivated in the region spanning modern-day Iran and Central Asia for over nine thousand years, among the oldest cultivated nut trees, with archaeological evidence from the Zagros Mountains region. They held real cultural prestige across ancient Persian, Middle Eastern, and later Mediterranean societies, appearing in royal gardens and considered a delicacy suitable for nobility.

Iran remains one of the world's largest pistachio producers today, a direct continuity from its ancient cultivation origins, alongside significant modern production having also developed in California over the past century.

What clinical research actually shows

Pistachio has an unusually broad and consistent evidence base spanning both cholesterol and blood sugar markers, evaluated across multiple independent meta-analyses. A meta-analysis of 12 randomized controlled trials found pistachio consumption significantly reduced total cholesterol, LDL cholesterol, and triglycerides. Separately, a meta-analysis of 8 RCTs found pistachio consumption significantly reduced fasting blood sugar and insulin levels in people at high cardiovascular risk.

A third, broader meta-analysis found significant improvements across an even wider range of markers — fasting glucose, cholesterol ratios, HbA1c, insulin, and systolic blood pressure — while confirming no adverse effect on body weight. This consistency across multiple independently-conducted meta-analyses, spanning both major cardiometabolic outcome categories, is genuinely comparable to walnut and almond's evidence bases elsewhere in this Atlas, which is what earns pistachio a "Strong" rating.
