While fatty fish has been a dietary staple across coastal cultures worldwide for millennia, the specific scientific concept of "omega-3 fatty acids" as a distinct, health-relevant nutrient category traces to 1970s Danish research studying the notably low rates of heart disease among Greenland Inuit populations, whose traditional diet was extremely high in fatty fish and marine mammal fat.

Omega-3 fatty acids have consistently strong, well-replicated evidence for one specific effect: lowering triglycerides. Multiple independent meta-analyses confirm significant, dose-dependent triglyceride reduction, a well-established and uncontested finding used clinically for hypertriglyceridemia management.

For the broader, more publicly recognized claim — preventing heart attacks, strokes, and cardiovascular death — the evidence is genuinely more contested. A large meta-analysis of 19 trials (97,709 participants) found omega-3 supplementation was not significantly associated with reduced all-cause mortality, cardiac death, heart attack, or stroke overall. This stands in real tension with the landmark REDUCE-IT trial, which found a specific high-dose, highly purified prescription EPA formulation reduced major cardiovascular events by 25% in a specific high-risk population — a genuinely different product and population than typical over-the-counter fish oil. This split is why "Moderate" is the accurate, nuanced rating.
