Eucalyptus oil has been used by Aboriginal Australian peoples for wound care and respiratory complaints for centuries, and was formally studied and adopted by European medicine beginning in the 19th century as eucalyptus trees were exported worldwide.

Modern research has concentrated on eucalyptol (cineole), eucalyptus oil's primary active compound, with clinical trials revealing a genuinely differentiated picture depending on the specific respiratory condition tested.

What clinical research actually shows

Eucalyptus oil's evidence varies meaningfully by condition. For ordinary cough associated with upper respiratory infections, a formal systematic review and meta-analysis found eucalyptus products safe, but concluded their "efficacy is minimal and of uncertain clinical importance." For COPD specifically, the evidence is considerably more substantial: a landmark double-blind trial following 242 patients for six months found cineole reduced exacerbations by 38.5-42.8% compared to placebo, alongside 1.8 days faster recovery during flare-ups.

A genuinely important safety concern must be flagged clearly: eucalyptus oil is toxic if ingested in more than trace amounts, and a documented case report describes a fatal poisoning after a man accidentally consumed 15mL, mistaking it for cough syrup. Given a differentiated picture — minimal benefit for ordinary cough, but substantial evidence for COPD exacerbations specifically — combined with real ingestion toxicity, "Limited-Moderate" reflects both the evidence and the necessary safety caution.
