A systematic review and meta-analysis, following PRISMA reporting standards and pooling 9 randomized controlled trials (414 patients), found no significant effect of bitter melon over placebo on fasting blood glucose, HbA1c, cholesterol, blood pressure, or any other metabolic marker examined. The review's title states this directly: "The metabolic effect of Momordica charantia cannot be determined based on the available clinical evidence."

Individual trials tell a genuinely inconsistent story — one 12-week randomized trial found fasting glucose improved but HbA1c remained unchanged, while a separate Korean prediabetes trial found modest positive effects on post-meal glucose handling. Given the most rigorous, PRISMA-based meta-analysis available found no significant effect on any primary outcome, "Limited" is the accurate, evidence-led rating.
